Seek Cutoff Part For a Simplex Lower Action

Hello -- I need a part for a Simplex lower action.  I need the wooden
box that sits over the stack cutoff slider.  It should have a hose
opening on it for an 1-1/2-inch ID hose.  Please let me know if anyone
has this part, as I'm pressed for time to complete this piano.
I attached a picture for clarification.
