Mandolin Bass Format Music Box

The DeBence Museum has a Mandolin Bass Format Music Box which over
the years has become reluctant to start.  Then for a while, if you
flipped the speed control fan with your finger, it would play.  Over
time this became less reliable, and finally no matter what you do, it
will not play.

I am no music box expert, but a full check shows no obvious problems
except that, as soon as you load the speed control worm set, it locks
up. It spins freely until you load it.

Yes, I checked for proper lubricant. Can someone with more experience
in this area point me toward the next step to fix this problem?
