Seek 5-cent Coin Slot Cover Plate

I am doing a nickelodeon conversion and would like to put a period
cover (escutcheon?) on the coin slot (1910-1915).  It should say 5¢
prominently.  It should be just the size of a nickel to keep out
quarters.  Would also like a return slot for pennies or dimes that
drop out.  Does anyone sell something like this?
