Worn Ladder Chain in Music Roll Transmission

I was playing the Ampico today and it stopped midway through the roll.
This had happened, oh too often, and I wasn't concerned -- the ladder
chain was off the sprocket.  I put it back on, finished the roll, and
on rewind it stopped again.  I put the chain back, played another roll,
and it stopped again!

I got frustrated and took off the chain.  It actually was warped; when
I held it straight, it looked like a curve from a road, tilted to the
center.  Not a problem -- I got some new chain and put it in place.

I was just wondering, how often do the ladder chains fail like this?
(Since replacing the chain, I played the piano for about an hour
straight and no more problems.  :))
