Making an Organ Crankshaft

I know I have seen plans for making up a crankshaft for a Wurlitzer
104/105 out of common flat and round stock, but I have spent the last
two days trying to find the document (normally, I keep _everything!_)
and I can't, so I have to give up and ask!  My wife says that all
guys hate to ask for directions.  I just _don't_ understand what she
means...   ";-)

Can anyone point me toward the plans?  I know it's a fairly simple
operation of some light machining and some welding, both of which I'm
equipped to do.  Where did I see those plans?
