If you can adapt a standard die to the book punch, your best bet would
probably be Roper Whitney. They make all kinds of punch die sets for
manual and machine punches. In addition to the custom clearances for
metal, they can custom fit a die to the clearance required for other
materials.
There are two companies (I forget the names at the moment) that make
ejector pins for injection molding, and these can be used as punches
with the necessary guides and dies, but your best bet is the plain old
Roper Whitney punches. The pin punches would probably need holders to
adapt them to the machine and they would cost as much as making the
required dies. If you need something custom, a single punch and die
set is no big deal for a round hole.
Most simple old machines used simple turned dies and punches which can
easily be made of hardenable steel. If you are just punching cardboard
you can probably just drill and ream the die and turn the punch and
harden them. If you need closer clearance, for thinner material, they
need to be ground to fit. A tool and die maker should not charge much
for such a simple set.
