The easiest way I have found to replace these when they're missing or
broken, is to borrow a pair in good condition (or take them off my own
piano) and have about any competent local jeweler cast duplicates and
plate them if desired.
Be aware too, that almost all reputable pawnshops have a jeweler on
staff who can do this. Much of what they do is to manufacture custom
jewelry and so they have all the casting and plating equipment
necessary.
A few years ago I had several sets made up, but have used or given them
away and haven't needed any since. The machine screws that hold them
on, with the proper head and thread and of appropriate length, were
always more difficult for me to locate.
If you don't head for Tiffany's to have it done, it's not expensive.
